This patient summary means to discuss key recommendations from the Society of Thoracic Surgeons (SCA), the American Society of Extra-corporeal Technology (AmSECT), the Society of Cardiovascular Anesthesiologists (SCA), and the Society for the Advancement of Blood Management (SABM) for patient blood management.
Overview
Overview
- Blood and blood product transfusion is common, particularly during heart surgery, and requires expert management.
- We will use the abbreviation CPB to refer to cardiopulmonary bypass (artificial replacement of heart function during heart surgery). The term “blood” will include blood products like plasma, clotting factors, and blood cells only.
- This patient summary focuses on transfusions during surgery.
Diagnosis
Diagnosis
- A thorough evaluation of the patient’s need for transfusion must precede surgery.
- It is possible to stimulate blood production prior to surgery.
Treatment
Treatment
- Standard protocols have been developed to optimize blood use and minimize blood loss.
- The many medicines that affect the blood (like “blood thinners”) will be reviewed and adjusted as needed.
- The specific blood product (or products) that might be needed will be identified and made ready.
- Adjustments to the CPB machine and its connections to the patient will optimize its use for each patient.
