Just published March 3rd, 2026, the American Association of Clinical Endocrinologists’ newest consensus statement on Algorithm for Management of Adults With Type 2 Diabetes – 2026 Update.
This update provides evidence-based visual guidance in graphic algorithms and a summary of evidence and considerations to assist health care professionals with the diagnosis and management of adults with prediabetes and diabetes mellitus in shared decision making to improve care.

The algorithm is divided into sections that outline the principles for: the management of adults with type 2 diabetes, management of prediabetes and prevention of progression to type 2 diabetes, diagnosis and classification of diabetes mellitus, treatment of cardiovascular risk factors, dyslipidemia and hypertension, comorbidities- and complication-centric and glucose-centric approaches for glycemic control with pharmacotherapy, and insulin initiation and titration.
Also included are updated tables that summarize the benefits and risks of medications for type 2 diabetes and obesity. Updated immunization guidance is provided that summarizes vaccine recommendations from the Advisory Committee on Immunization Practices of the U.S. Centers for Disease Control and Prevention as they currently stand at the time of publication.
This 2026 update emphasizes lifestyle modification and treatment of overweight/obesity as key pillars in the management of prediabetes and type 2 diabetes.
The update also includes a new algorithm, the diabetes classification algorithm, which is intended to remind clinicians to consider other cases of diabetes mellitus beyond type 2 diabetes, that can inform further investigation of secondary causes, natural history, and management strategies.
