The 2025 American College of Clinical Pharmacy (ACCP) Annual Meeting is quickly approaching. Set for October 18 through October 21, in Minneapolis, Minnesota, the 2025 ACCP annual conference is expected to draw in more than 1500 clinical pharmacy professionals from around the world.

With four days packed full of educational sessions, poster presentations, business meetings, and networking forums, the 2025 ACCP Annual Meeting is an excellent opportunity to learn about the latest in the clinical pharmacy field.

Registration is now open for interested parties who have yet to register for this year’s conference. The ACCP also welcomes non-members to register.

Managing Risk: Transition in Antithrombotic Therapy and Lipid Medication Management

  • Description:  Attendees will receive an overview of approaching peri-procedural antithrombotic medication therapy, as well as how to determine the most appropriate patient-specific therapeutic monitoring plan throughout the different stages of patient care. 

Transplant PRN Focus Session – The Eras Tour – SOT Edition: Optimizing Pharmacotherapy During Healthcare Transitions of Care

  • Session Description: This session will feature discussions on the management of solid organ transplant recipients and patients with chronic kidney disease going through various transitions of care, which require supportive care, wherein pharmacists have an important role to play.

Ensuring Impact: Optimizing Transitions of Care Activities in Patients with Cardiovascular Disease

  • 11:15 AM – 11:35 AM
  • Description: This session takes place as part of the larger, Cardiology PRN Focus Session that runs from 10:15 AM through 11:45 AM. During the sessions, attendees learn about four diverse cardiovascular cases. Particularly, this speaker’s selection focuses on the most effective means of conducting transitions of care activities in cardiovascular patients. The cases presented during the overall presentation demonstrate examples of patient-specific cases where evidence and/or recommendations are unclear.
