The 2025 American College of Clinical Pharmacy (ACCP) Annual Meeting is quickly approaching. Set for October 18 through October 21, in Minneapolis, Minnesota, the 2025 ACCP annual conference is expected to draw in more than 1500 clinical pharmacy professionals from around the world.
With four days packed full of educational sessions, poster presentations, business meetings, and networking forums, the 2025 ACCP Annual Meeting is an excellent opportunity to learn about the latest in the clinical pharmacy field.

Clinical pharmacists are vital in transitions of care by ensuring continuity across the care continuum through maintaining consistency and identifying potential discrepancies in patient care. - Session Description: This session will focus on the role of comprehensive medication management in addressing and identifying medication errors and optimizing therapy during care transitions.

- There’s a Toolkit for That: Applying the CMM in Primary Care Study Instruments to Care Transitions
- 2:15 PM – 3:45 PM
- Session Description: Attendees will learn about creating pathways for safe transitions of care for patients receiving parenteral nutrition, along with learning about managing nutrition-related product shortages, and formulating a nutrition plan using ASPEN and ESPEN critical care nutrition guidelines.

Managing Risk: Transition in Antithrombotic Therapy and Lipid Medication Management

- Description: Attendees will receive an overview of approaching peri-procedural antithrombotic medication therapy, as well as how to determine the most appropriate patient-specific therapeutic monitoring plan throughout the different stages of patient care.

- Session Description: This session will feature discussions on the management of solid organ transplant recipients and patients with chronic kidney disease going through various transitions of care, which require supportive care, wherein pharmacists have an important role to play.

Ensuring Impact: Optimizing Transitions of Care Activities in Patients with Cardiovascular Disease

- Description: This session takes place as part of the larger, Cardiology PRN Focus Session that runs from 10:15 AM through 11:45 AM. During the sessions, attendees learn about four diverse cardiovascular cases. Particularly, this speaker’s selection focuses on the most effective means of conducting transitions of care activities in cardiovascular patients. The cases presented during the overall presentation demonstrate examples of patient-specific cases where evidence and/or recommendations are unclear.
