A woman in her 60s presented to dermatology for evaluation of a dry mouth with associated discomfort of 3 months’ duration. Her medical history was notable for prior bilateral breast cancer, colon cancer, and goiter. Her family history was notable for 2 third-degree relatives who were also diagnosed with multiple different tumors and a child who had been diagnosed with a hepatoblastoma. On physical examination, hypertrophy of dorsal tongue papillae in a honeycomb pattern was observed (Figure) along with papillomatous growth of the gingiva. No other dermatologic features were noted on a complete skin examination. Genetic testing had revealed a substitution C>A in the region c.287 of the gene PTEN, which in combination with the clinical findings, confirmed the diagnosis of Cowden syndrome (CS).
