A 61-year-old male with diabetes and hyperlipidemia reported blurry vision in his right eye since waking up. He exhibited no other neurologic signs. The night before, he had injected his first dose of semaglutide. The visual acuity was 20/25 in both eyes. The right eye had a relative afferent pupillary defect. Fundus examination demonstrated mild retinal whitening and edema in the perifoveal area, with sparing of the nasal macula (Figure). Spectral-domain optical coherence tomography revealed inner retinal hyperreflectivity temporal to the fovea and preserved retinal architecture nasally (Figure), consistent with central retinal artery occlusion (CRAO) with sparing of the nasal macula due to cilioretinal artery perfusion.
