Anemia

  • Hgb <13 g/dL (Male)
  • Hgb <12 g/dL (Female)

Severe Renal Disease

  • GFR <30 mL/min or dialysis-dependent

Age ≥ 75

Any Prior Hemorrhage Diagnosis

  • Ex: GI bleed, intracranial hemorrhage
