Just published July 21st, 2026, the American College of Chest Physicians’ newest guideline on Management of Adult Bronchiectasis.
This guideline aims to harmonize care for patients with bronchiectasis, reduce practice variation, and optimize outcomes for this increasingly prevalent, high-burden disease

- For the management of acute exacerbations of bronchiectasis, the guideline suggests targeted antibiotic therapy based on microbiological culture results
- For the treatment of acute exacerbations of bronchiectasis, the guideline suggests either longer duration antibiotic therapy or shorter duration antibiotic therapy depending on clinical response
- For patients with bronchiectasis and a first isolate of Pseudomonas aeruginosa, the guideline makes no specific recommendation for or against eradication therapy compared to no eradication therapy
- For patients with bronchiectasis experiencing frequent exacerbations, the guideline suggests long-term inhaled antibiotic therapy over no long-term inhaled antibiotic therapy
- For adults with frequent exacerbations of bronchiectasis, the guideline suggests long-term macrolide antibiotic therapy
- For adults with frequent exacerbations of bronchiectasis, the guideline suggests long-term brensocatib therapy
- For adults with frequent exacerbation of bronchiectasis, the guideline suggests against long-term atorvastatin therapy
- For patients with bronchiectasis and hemoptysis, the guideline suggests tranexamic acid
- For patients with bronchiectasis, the guideline suggests against the use of rhDNase for airway clearance
