The 2026 report from the Global Initiative for Chronic Obstructive Lung Disease (GOLD) was recently released. This annual report focuses on the diagnosis, management, and prevention of COPD. Since 2001, the GOLD report has helped guide healthcare professionals in effectively managing patients living with COPD.

The 2026 GOLD report, the sixth major revision since the original report was published, features many updates and new elements that we outline below in today's article. Links to summary and full-text versions of the report are provided below, for your convenience.

Rundown of Key 2026 Gold Report Additions and Updates:

New Content:

  • Three new appendix sections were added: Abbreviations, Overview of the Evidence: Pharmacotherapy, and Overview of the Evidence: Non-Pharmacological Therapy.
  • Figure 3.11 (Evidence Supporting the Use of Biologics in the Treatment of COPD) is new and outlines evidence for the use of biologic therapy.
  • Two new figures (Figure 2.8: Factors that May Be Associated with COPD Underdiagnosis; Figure 2.9: An Algorithm for COPD Case-finding) were added to chapter 2.
  • A new section on disease activity was added to chapter 3’s introduction.
  • New figures were added to chapters 4 (figures 4.3, 4.4, and 4.7) and 5 (5.1, 5.2, 5.3, 5.5, and 5.6).
  • Chapter 6 is a new chapter on artificial intelligence and emerging technologies. 
  • An overall total of 14 figures are new to this year's update.

Updated Content:

  • The 2026 GOLD report update shifted a significant amount of content to Appendices 1, 2, 3, and 4 to reduce repetition and improve flow.
  • Associated abbreviations (as seen in the newly added appendix) are used throughout the report to improve flow and readability.
  • The Burden of COPD section in chapter 1 had epidemiological statistics and references updated.
  • Screening and Case-finding, in chapter 2, was updated based on newly available information.
  • Chapter 3 also received updates in two sections: Treatment Algorithms and The Management Cycle. Additionally, chapter 3's vaccination recommendations for people living with COPD were updated with the most recent information on influenza and RSV vaccinations.
  • Chapters 4 and 5 were completely revised.
  • Factors used in defining Gold A, B, and E were updated based on the latest findings. 
  • A refreshed review of references was conducted, and 330 new references were added.
